| 内容記述 | 要 旨 過去５年間の精神科での発達障害児・者に対する看護研究から，病棟での効果的な看護を抽出した．医学中央雑誌WEB版で，児童の14件と成人18件で，目的に合致した文献９件，14件を対象とした．児童は児童精神科病棟４，精神科病棟・児童思春期外来が各２，児童相談所１件で行われ，看護師や看護記録を対象にした研究が５件で，事例研究４件のうち，病棟で行われた３件をさらに分析した．成人と比べ退院先と養育者への支援が一貫して意識されている．成人の文献は，すべて事例研究で，デイケアでの就労支援１件以外は病棟で行われ，入院中の７件，退院した６件に分けてさらに分析した．入院中の文献は期間が短く，成育歴がなく，問題行動のみに注目していた．効果的な支援は病棟全体での安心安全な対人環境を受け持ち看護者を中心に整え，必要な成長発達支援とするには，特に成人事例でトラウマ・インフォームドケアの視点での他者との楽しい心地よい体験が重要で，看護師同士のサポートが必要である． Abstract This study extracted articles related to effective nursing in psychiatry for children and persons with developmental disabilities over the past five years. A total of 23 articles that met the inclusion criteria were obtained from Ichushi-web. Participants comprised 14 children and 18 adults. For children, four child/adolescence psychiatric wards, two psychiatric wards/child adolescent ambulatory care, and a child consultation center were selected. Five studies were carried out on nurses and nursing records; of these, four were case studies. The three case studies in psychiatric wards were further analyzed. Compared to nursing for adults, support for discharge destinations and caregivers was consistently conscious. The articles involving adult participants included case studies from the wards, except for one article related to employment support provided for day treatment, and was further analyzed by classifying it into 7 cases during hospitalization, and 6 cases after discharge. The studies carried out in hospital settings were of short duration, had no history of growth, and focused only on problematic behaviors. The fun comfortable experience with others in trauma-informed care wards are important, especially for adult cases, and the effective support between nurses is necessary to create a reliable and safe interpersonal environment in the whole ward, with necessary growth development support for the patients. |
